What is NDIS (National Disability Insurance Scheme)?

 

NDIS is an initiative by the Australian government to provide support to Australians experiencing disability, their families and carers.

This initiative provides Australians under the age of 65 who experience permanent and significant disabilities with the reasonable and necessary support they require and to assist them in their daily lives, including community involvement, education, employment, health and wellbeing.

The NDIS takes a lifetime approach, investing in people with disability early, to improve their outcomes later in life.

Are you/your loved one aged between 7 and 65?

If yes, go to the next question.

If you have a child aged below 7, contact us to find out more about help for children under 7.

If you or your loved one is over 65, only those under 65 can access the NDIS. You may be able to access other supports. Find out more about NDIS and Aged Care, contact us.

Do you live in Australia and have Australian residency?

If the answer is yes, you live in Australia and are an Australian citizen or have a Permanent or Special Category Visa, you can go to the next question.

If no, then you may be able to access other supports. The NDIS is only available to:

- People who live in Australia

- Australian citizens

- Permanent Visa holders

- Protected Special Category Visa holders

Engaging as your Service Provider

NDIS participants are given the choice and control to choose the service provider of their preference. As a registered NDIS service provider, Concept Care is highly responsive to your requirements by providing you with a dedicated care consultant who will listen and understand your needs and wants. A suitable caregiver is then allocated to you or your loved one to ensure an exceptionally tailored service is provided to help achieve your goals.

Our Services

Personal Care

Concept Care provides assistance with showering, dressing and toileting. This includes both stand-by and full assistance. Our caregivers are experts in all aspects of personal care and safe manual handling procedures.

Flexible/In-Home Respite Care

Concept Care provides respite services to enable you and your carer or family member to have a well-earned break. In-home respite services means carers and family members can rest easy knowing that their loved one's routine remains uninterrupted and they are in an environment which caters to their needs. Respite care can be delivered either regularly or once-off depending on your needs.

Complex Needs Support

We provide assistance with hoisting, PEG feeding, continence and bowel care management including catheter care. Extensive training is provided for all services listed above. For any service listed outside of these, training can be arranged with a clinical nurse educator so we can meet your individual specific needs.

Domestic Assistance

Assistance with a wide range of domestic duties including cleaning, dusting, vacuuming, mopping, washing, ironing and other household tasks as required. As it is your home, you decide on the domestic duties which you need and how our staff performs them.

Meal Preparation

Assistance with cooking and preparing meals. Our staff has been trained in safe food handling practices and can assist you with all your meal preparation needs. Choose your own meals and get our employees to prepare them.

Gardening Assistance

Concept Care provides expert gardening and yard work assistance such as regular maintenance visits, hedging & pruning, weeding, planting, mulching, lawn mowing & edging.

Community Access

Assistance to attend family and community events, go shopping, or while pursuing hobbies and interests or even when travelling on holiday. Assistance can also be provided when attending work, school or your place of study. We can even help you choose or design a community-based event that suits your needs.

Travel & Transport

Provide assistance to gain independence and support with travelling and transport to and from social and community participation, e.g. going out for a coffee with friends or playing sport.

Companionship

Our caregivers provide healthy social interactions and activities that promote good physical and cognitive health. They also cater to an individual’s personal preference, encouraging activities and conversations that they love and enjoy. Companionship activities include:

  • Conversation
  • Company over meals
  • Social outings
  • Outdoor activities like walking or gardening
  • Crafts, scrapbooking, knitting and other creative outlets
  • Games to promote mental stimulation
  • Discussion of current and historic events to help sharpen memory
  • Reminiscing as an enjoyable pastime
